2008 Volleyball Application FormsHi Everyone.
Attached are the Summer 2008 CCU volleyball forms. I have broken the forms into three files. (See attachments.) The "2008 General Information.pdf" file consists of general information about the league. The "2008 Waiver.pdf" file is the waiver form. You will need Adobe Acrobat reader to open and print these files. You can download the free reader from Adobe's website at http://www.adobe.com/products/acrobat/readstep2_allversions.html , select your computer type, and operating system. The "2008 Volleyball Application.doc" file is a Microsoft Word document that can be printed and mailed along with the entry fee or edited and emailed to me electronically while the check is mailed separately. 1. Mailing location for the application forms.
2. Stated penalty for participants skipping out of fellowship as requested from the 2007 wrap-up meeting. 3. Waivers are now due by 8:00pm May 11, 2008, the cut-off date. In Christ,
