Summer 2009 CCU Volleyball FormsHi Everyone.
Attached are the Summer 2009 CCU volleyball forms. I have broken the forms into three files. (See attachments.) The "2009 General Information.pdf" file consists of general information about the league. The "2009 Waiver.pdf" file is the waiver form. You will need Adobe Acrobat reader to open and print these files. You can download the free reader from Adobe's website at http://get.adobe.com/reader/otherversions/ , select your operating system and language. The "2009 application.doc" file is a Microsoft Word document that can be printed and mailed along with the entry fee or edited and emailed to me electronically while the check is mailed separately. 1. Skills clinic moved from the first day of the league to the afternoon (1:00 to 5:00pm) of the Sports kick-off.
